The template is located within the patient demographics box. Click the 'red telephone' icon to open.
Assessment
Triage - tick to record this is a triage.
Consultation button - direct access to the General Consultation template for prescribing & leaflets etc.
Ambulance triage - has the patient been triaged by the ambulance service.
Telephone Follow-up - tick to record the consultation as a follow-up.
Video Conference/Skype - document a video consultation. The button will launch Skype on the login page.
3rd party - if speaking with a family member / carer of the patient, record their name
Problem - Lists symptoms and patient's request
Vitals & Lifestyle - view history of previous readings (e.g. click into the BP box and view historic readings in the right side of the template). Use the template to also record new data.
Diaries / Scores - opens all scored assessments and patient diaries that we have available. These can be printed and left at reception for the patient to collect or forward the URL link via text message.
Investigations - Links to phlebotomy etc. to view previous blood results or other investigations and also provides a link to electronic pathology requesting (ICE/TQuest etc.).
Impression
Record your impression either by read code or free text.
Management
Plan - write the plan going forward
Advice - tick to record advice given. Use the pencil icon to write the advice given.
Leaflets & Med3 - print, text or email leaflets, print a Med3 or generate letters
Formulary - access the main formularies including paediatric medicine for prescribing
Criteria - referral criteria and future care planning
Safeguarding - record if any safeguarding issues have been identified
Follow-up - arrange a follow up. Use the follow-up template or book an appointment or home visit
Failed Encounter
Code the failed encounter and type the message given for the patient. Use the preset button to select from a list of frequently typed phrases.
Don't forget to ok the template and save the record when finished.
