Alternatively,  add the template to the F12 launcher.

Assessment

Record that you have discussed confidentiality. If you need to change the confidentiality status of the encounter click the button and select accordingly:

Read code -  pregnancy information, smoking, sexual activity and sexual health.  

Vitals - enter relevant values into the boxes i.e. BP, weight etc.

UK MEC Risk - to assign a level of risk:

There are various tabs to select from - C-OCP, POP, Implant, Injection, LNG-IUS, IUD

Either use the tick boxes along the top to assign the overall MEC Risk or run through the individual risks with the patient:

Menstrual diary - print if required - click print at the bottom.  The Patient name and address will also print on it:

Click the buttons for the STI/BBV and Chlamydia screens 

Management 

PIL & advice - issue patient information leaflets and give LARC advice.  Don't forget that any red stars indicate that a QOF payment is linked to the field.  

Record that advice has been given and type any detail onto the pencil icon.
