Ardens has carefully considered the risks/benefits of producing a QRISK3 calculator that works from within EMIS Web and SystmOne. The obvious benefit is ease of accessing this calculator for clinicians. There are, however, significant risks with capturing very specific coded data and using EMIS Web/SystmOne to perform complex calculations based on it that may result in a patient’s management plan being altered.

An example of this is the MHRA alert relating to the QRISK2 calculator in 2016. This incident highlighted the difficulties in risk estimation and required GP practices to undertake additional work (via a National Enhanced Service) to address the issue. The lessons learned from this event underscore the necessity for caution with any new risk assessment tool.

In light of this, and following a review all of the clinical safety issues, we feel that some of the current risks are not possible to adequately mitigate within the current functionality of the clinical systems. We remain dedicated to developing resources that meet the highest safety standards and have therefore taken the decision not to develop an integrated calculator at this time, but to direct users to the online calculator.

The current options therefore, are either:

  • Use the existing inbuilt EMIS or SystmOne QRISK2 calculator, which adds the QRISK2 numeric code and value automatically
  • Use the link to the QRISK3 online website calculator and to manually add all the risk factors, and then add the QRISK3 value on to the EMIS or SystmOne template separately.

NB: If you are using the QRISK3 online website calculator, please be cautious about doing this due to the risk of human error when asking the patient for risk factors, checking their records and inputting the values individually online.
