Ardens has created a template to support practices in recording information during health checks for patients aged over 75.
Access the template in one of the following ways:
- Go to Auto-Consultation> ardens HEALTH CHECKS> Over 75's Health Check
- Type the name of the template using the bottom left search in SystmOne
- Add to your F12 favourites
The template has been broken down by pages along the top or using the blue hyperlinks on the Home page. Navigate to the page required.
Home page
Lists the relevant codes to document an Over 75's Health Check, declined, unsuitable or invitation.
An assessment form can be printed if a home visit is required. The form will list some relevant information from the patients record such as medical conditions and current medications.
The Physical Assessment page provides the opportunity to record data such as weight, height, BMI and waist circumference, BP, pulse etc. Link to QRisk2 calculator or QRisk3 (ClinRisk website). There are dedicated free text boxes for recording information about any ENT, dental & oral, cardiovascular, respiratory and other problems. Use the icons to the right of the free text boxes to enter preset text.
The Psychological page provides screening for memory and depression using scored assessments.
The Lifestyle page is used to record a full alcohol AUDIT or AUDIT-C and smoking questionnaire as well as any interventions or referrals that may be made as a result.
The Functional, Social & Environment page can be used to record details of personal care, frailty & falls, carer information, housing situation, diet etc. with links to other templates such as Fuel Poverty.
The Medication page provides a full list of current medications and shows any drug alerts, monitoring or bloods tests overdue. There is a link to the Drug Review template.
The Planning page provides leaflets and documents to assist in making decisions about future care, with links to DNACPR and Respect Plans. A printable word document which will merge data recorded during the health check, into a Care Plan Summary. Don't forget to click Ok on the template first before launching the document. This will ensure the data will merge through.
You may also record seasonal flu and pneumococcal vaccination advice with links to the vaccination templates.
Reports
There are various reports to monitor the Over 75's Health Check activity in the last month, send first/second invitations and provides performance indicators showing any missing elements of the health checks conducted.
